Pre-approval requests can be made to determine if your pet’s condition(s) may or may not be eligible for coverage. If your pet has a condition and you would like to know if it would be eligible for coverage before considering treatment, please email us a completed pre-approval form along with a copy of your pet’s complete medical history. Once reviewed, we will email you a pre-approval determination form with the results of our review.
